Minneapolis VA Health Care System (VAHCS) is a teaching hospital providing a full range of patient care services with state-of-the-art technology, as well as education and research. Family Practice or Internal Medicine trained primary care provider is sought for a VA clinic in Maplewood, Minnesota part of the Minneapolis, MN VA health care system. Our facilities include an updated fully-equipped medical clinic with a lab, x-ray and telemedicine capabilities. Our physicians serve the primary care health needs of Maplewood and surrounding areas using a team approach with nursing and onsite clinical pharmacist. Mental Health services within the clinic help to provide comprehensive care to the Veterans served in this VA Community Clinic. Take time with your patients. Visits for established patients are 30 minutes and for new patients 60 minutes.

Providers will perform outpatient duties and management of scheduled and unscheduled patients. An assigned panel of patients is determined by the VHA PCMM software package which is specific to their practice and setting. Focus on care include the chronic disease initiative areas of CHF, COPD and DM, mental health collaboration in the PC setting, MOVE program focusing on weight loss, diet and activity as well as health promotion/disease management.
Work Schedule: The tour of duty for this position is typically Monday - Friday, days Tour TBD at time of hire. Additional information may be provided during the interview process.
