Develop personalized care plans in collaboration with clients and families, ensuring each plan aligns with specific goals, preferences, and available resources. \Provide ongoing counseling and emotional support to seniors and their families, helping them navigate life transitions, such as aging, illness, and loss. \Advocate for clients' rights and facilitate access to community resources, ensuring that they are informed and receive the necessary support services. \Engage with interdisciplinary teams to enhance overall care, regularly communicating updates and plans of action with nurses, caregivers, and other health professionals. \Conduct regular home visits to monitor clients' well-being and living conditions, identifying any concerns or issues that may affect their safety and quality of life. \Facilitate family meetings, assisting in conflict resolution and enhancing communication among family members involved in the client's care. \Educate clients and families about community resources, support groups, and programs available to enrich social engagement and quality of life. \Maintain accurate and detailed documentation of assessments, care plans, and progress notes in compliance with organizational and ethical standards. \Stay current with best practices and regulations in social work, participating in professional development and networking opportunities. \This role focuses primarily on seniors who are facing complex healthcare challenges due to aging, chronic illnesses, or terminal conditions. You will be instrumental in bridging communication between healthcare providers and elderly clients, ensuring a seamless continuum of care that addresses both physical and emotional needs.
