Customer Service Facilitators

Seeking a compassionate Service Facilitator for the CCS Program, responsible for assessing client needs, developing personalized service plans, and coordinating community resources to promote independence and well-being.
Key Duties Include Conducting functional and needs assessments.
Creating and implementing person-centered service plans.
Connecting clients with community resources and services.
Facilitating team meetings and collaborating with families, providers, and support networks.
Maintaining accurate documentation and client records.
Advocating for clients using trauma-informed and strengths-based approaches.
Qualifications And Skills Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or related field.
Preferred experience working with individuals, families, or vulnerable populations.
Strong communication, organization, and relationship-building skills.
Ability to manage documentation, coordinate multiple services, and work independently.
Valid driver's license, reliable transportation, and willingness to travel within the community.
