Registered Nurse Case Manager (RNCM) – Hospice

Schedule: Full-Time | Monday–Friday, 8:00 AM–5:00 PM | Occasional evenings/weekends

Vitality Hospice & Palliative Care is a clinician-owned and operated organization dedicated to providing exceptional care at the end of life. We are seeking an outstanding Registered Nurse Case Manager (RNCM) who is passionate about making a difference in the lives of patients and their families.

Conduct patient admissions and ongoing nursing assessments

Develop and implement individualized plans of care based on each patient's needs and goals

Collaborate closely with hospice physicians and the interdisciplinary team (IDT)

Supervise and coordinate care with home health aides, social workers, and chaplains

Ensure timely and accurate documentation in compliance with hospice standards

Current Registered Nurse (RN) License in New Jersey (required)

Clinical nursing experience (hospice or home health preferred)

Valid driver's license, reliable vehicle, and current auto insurance

Ability to travel within the assigned service area
