Territory/Location: Travel-based position supporting multiple home health locations. Position Overview The Lead Physical Therapist provides high-quality physical therapy services and clinical guidance across our home health locations. This is a permanent, full-time W-2 position designed for clinicians who enjoy travel, flexibility, and the opportunity to make a meaningful impact across multiple communities. Team members complete 13-week minimum assignments at designated locations and receive company-paid housing, transportation, and travel expenses to and from each assignment . The Lead Physical Therapist serves as a clinical resource, promotes best practices, and helps ensure exceptional patient outcomes while supporting operational and clinical excellence throughout the organization.

Serve as a clinical preceptor and resource for rehabilitation staff across assigned locations.

Support mentoring, onboarding, and development of therapy team members as needed.

Participate in case conferences, care planning, and quality improvement initiatives.

Assist operational and clinical leadership in ensuring consistent delivery of therapy services across multiple markets.

Qualifications Active Physical Therapist license in the practicing state or ability to obtain licensure in assigned states as required.

Master's or Doctorate degree in Physical Therapy from an accredited program.

Minimum of one (1) year of experience as a Physical Therapist; home health experience strongly preferred.

Valid driver's license and ability to travel extensively between assignments.

Ability and willingness to complete 13-week minimum assignments at various home health locations based on organizational needs.
