Social Worker – Skilled Nursing Facility (SNF) Facility: Acclaim Rehab and Nursing Center

Location: Jersey City, NJ

Schedule: Full-Time | Monday–Friday, 9:00 AM–5:00 PM

Salary: $65,000 annually

Join Our Team! Acclaim Rehab and Nursing Center is seeking a compassionate and dedicated Social Worker to join our Skilled Nursing Facility (SNF) team. The Social Worker will provide social services, resident advocacy, psychosocial support, care planning, and discharge planning for residents and their families.

Responsibilities Provide social work services to residents in a SNF and long-term care setting.

Complete psychosocial assessments, social histories, progress notes, and documentation.

Participate in MDS assessments and care plan meetings.

Develop and coordinate individualized care plans and discharge plans.

Advocate for residents and support resident rights, preferences, and needs.

Assist residents and families with Medicare, Medicaid, insurance, financial assistance, and community resources.

Coordinate referrals and services with hospitals, healthcare providers, and community agencies.

Provide support to residents and families throughout the rehabilitation and long‑term care process.

Collaborate with nursing, rehabilitation, medical, and other interdisciplinary team members.

Participate in Quality Assurance, Utilization Review, Facility surveys, and committees as assigned.

Maintain accurate and timely Social Services documentation.

Address resident and family concerns and assist with resolving psychosocial and discharge‑related issues.

Requirements Bachelor’s Degree required in Social Work, Sociology, Psychology, Rehabilitation Counseling, Special Education, or a related field.

Master’s Degree in Social Work (MSW) preferred.

1-2 years experience in skilled nursing, long‑term care, rehabilitation, or healthcare preferred.

Knowledge of MDS, care planning, discharge planning, Medicare/Medicaid, and resident advocacy preferred.

Strong communication, organization, documentation, and interpersonal skills.
