Competitive Pay: $40.33 - 50.33 per hour, based on experience
Schedule: Full-Time | Day shift available, 8 hour shift
Monitor resident wellness and respond to changes in condition as needed
Guide and support caregiving staff to maintain quality resident services
Partner with community leadership to coordinate resident care needs
Complete assessments and update individualized care plans with resident and family input
Coordinate appointments, referrals, transportation, and care-related communication
Oversee medication-related services and ensure regulatory compliance
Document incidents, investigate concerns, and follow through on action plans
Collaborate with residents, families, and team members to promote safety and well-being
Current and active RN license in New Jersey (required)

• 1+ years of nursing experience preferred
