Sierra Valley RehabCenter is a skilled nursing facility in Porterville, CA, dedicated to providing compassionate, resident-centered care. We are currently seeking Licensed Vocational Nurses (LVNs) to join our dynamic clinical team. The LVN provides direct nursing care to residents under the supervision of a Registered Nurse or Director of Nursing. This includes administering medications and treatments, supervising Certified Nursing Assistants (CNAs), monitoring patient care, and ensuring adherence to all regulatory and facility standards.
Supervise and support CNAs in delivering quality care
Chart all care provided, resident responses, and any changes in condition
Participate in care planning, quality assurance initiatives, and staff development
Conduct rounds and ensure compliance with nursing procedures
Respond to emergencies and provide critical nursing interventions
Maintain accurate, timely documentation in compliance with state and facility policies
Promote resident rights, dignity, and confidentiality at all times
Current and unencumbered LVN license in the state of California
Graduate of an accredited LVN program
Prior experience in long-term care or skilled nursing preferred
Proficient in basic computer use and electronic medical records (PCC experience preferred)
Must be able to respond to emergency situations and provide hands-on care
Capable of working under pressure in a fast-paced healthcare environment
