Home Health Care Licensed Practical Nurse For Weekends (LPN)

Pride Health is seeking an experienced Registered Nurse (RN) to provide skilled home health services to patients throughout Monroe, West Monroe, Rayville, Columbia, Jonesboro, Hodge, Ruston, Bernice, Farmerville, Marion, and surrounding communities within a 50-mile radius. This position is responsible for delivering high-quality patient care, coordinating with physicians and interdisciplinary teams, completing timely clinical documentation, and supporting positive patient outcomes in the home setting.

Patient Care Setting: Home Health

Schedule: Monday-Friday, 8:00 AM - 4:30 PM

On-Call Requirement: Yes, weekday and weekend rotation

Mileage Reimbursement: Yes, reimbursed between patient visits (first patient to last patient)

Provide skilled nursing care in accordance with physician orders, agency policies, and individualized plans of care.

Communicate patient status updates, concerns, and significant changes to physicians and agency leadership in a timely manner.

Develop and implement patient-centered care plans to support optimal health outcomes.

Complete accurate and timely clinical documentation utilizing Homecare Homebase (HCHB) and agency systems.

Coordinate care with physicians, therapists, caregivers, and interdisciplinary healthcare teams.

Participate in weekly case conferences and collaborate on patient care planning and interventions.

Participate in weekday and weekend on-call coverage as assigned.

Current, unrestricted Registered Nurse (RN) license.

Previous home health experience.

Experience utilizing Homecare Homebase (HCHB).

Strong clinical assessment and patient care skills.

Ability to travel throughout the assigned service area.

Flexible and adaptable in a fast-paced healthcare environment.

Team-oriented approach to patient care.

Experience managing an independent home health caseload.

Knowledge of Medicare home health regulations and documentation standards.
