Home Health LVN (Shift care) - Part-time or Full-time

We are seeking Private Duty Licensed Vocational Nurses to join our team, which is committed to the highest standards of care, integrity and reliability.
Hallway Healthcare provides in-home nursing care to retired Nuclear and Federal Workers who have received benefits through EEOICPA or FECA. These workers have retired from Department of Energy facilities such as Lawrence Livermore National Lab, Sandia National Lab Lawrence Berkeley National Lab, Stanford Linear, etc. Unlike many home health agencies that require multiple short visits per shift, Hallway Healthcare typically assigns you to one patient per shift, allowing for more focused, consistent, and meaningful care.
Available Shifts: Full-time or Part-time Thursday, Friday, Saturday 7pm-7am or per diem for 7am-7pm

Teaching and Training Activities
Graduated from an accredited school of professional nursing.
Current Texas RN/LVN License
Support and contribute to quality improvement initiatives.
Perform nursing procedures in accordance with agency policies and procedures.
Work under the supervision and direction of a registered nurse (RN).
Educate patients and their families about disease processes, medications, plans of care, treatment options, and home care procedures as outlined in the plan of care.
Report any adverse findings or concerns to the physician and RN promptly.
Assist with activities of daily living, such as bathing, dressing, feeding and light housekeeping.
Transport patients to doctors appointments, grocery stores or pharmacies.
Coordinate and monitor patient care services to ensure optimal outcomes.
Follow infection control policies rigorously in all settings.
Document skilled visits accurately and in accordance with established guidelines. Documentation must be completed by the end of each shift. Light housekeeping and meal preparation.
Maintain comprehensive and up-to-date patient records in line with agency policies.
Environmental Elements
The role involves managing challenging interactions with staff, public officials and private representatives while interpreting and implementing departmental policies and procedures.
Employee will have to travel to a variety of patient homes and perform in conditions that vary greatly depending upon the client's home environment. Some homes will be clean, neat, and maintained at a comfortable temperature. Sufficient vision or other powers of observation, with or without reasonable accommodation, to enable the employee to review a wide variety of materials in electronic or hard copy form;
