This position is full time salaried position, working only weekends Friday, Saturday, Sunday. Eligible for full time benefitsThe Registered Nurse in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The Registered Nurse coordinates total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. All done within a Point of Care setting.Develops, implements, and updates patient plans of care in collaboration with physicians and the interdisciplinary team; health eligibility and completes comprehensive nursing assessments (including OASIS and medication reconciliation).quality clinical care; supervises and assigns tasks to home health aides and LPN/LVNs in compliance with state and federal regulations, including required supervisory visits.Accurately documents all patient care per policy and payer requirements; communicates timely with physicians, patients, families, and agency staff to ensure coordinated, effective care.contributes to performance improvement, survey readiness, patient safety initiatives, and reducing avoidable hospitalizations.call/weekend rotations.Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the clientYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Current and unrestricted RN licensure in state of practiceCurrent Driver's License, vehicle insurance, and access to a dependable vehicle or public transportationPreferred Qualifications:1+ years of clinical experienceCPR Certification or ability to complete within 90 days of hirePay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $70,832 to $106,248 annually based on full-time employment.
