Rehabilitation Counselor, Full Time

This position will develop and maintain current employer partner relationships as well as develop new relationships with businesses to coordinate services in the community, assist with securing employment, and provide retention supports to those actively employed.PRIMARY RESPONSIBILITIES:Obtain and review all required documentation and determine participant eligibility.Complete comprehensive needs assessment on all individuals referred with respect to contract policies and guidelines.Develop individual employment plans and assist participants in accessing the public and private resources to meet goals outlined in the plan.Provide intensive case management and advocacy for participants and their families.Help participants develop and strengthen work behaviors and employability.Establish effective relationships with area training institutions to increase participant’s skill levels and job market competitiveness.Promote continued education and lifelong learning to prepare participants for career advancement.Assist participants with obtaining and maintaining competitive employment.Distribute incentives and support services in accordance with agency guidelines.Identify possible areas for program improvement based on the need of participants.Maintain participant files; submit reporting forms, and other data as required.High school diploma or GED is required.With a high school diploma or GED, a minimum of 4 years of experience in a similar role is required.If associate degree in human services or related field, a minimum of 2 years of experience in a similar role is required.If bachelor’s degree in human services or related field, no additional experience is required.
