The university leads multiple partnerships and programs to provide equitable healthcare resources in a variety of south Los Angeles neighborhoods.Job Summary:The MoHOP Project Nurse Practitioner (NP) will support our DFM mission to ensure access to high-quality, patient-centered, equitable health care for Los Angeles County residents, with a special focus on medically under-resourced and uninsured racial/ethnic minority populations residing in South Los Angeles (SPA 6), Skid Row, and other regions of Los Angeles County. This position will support the CDU Street Medicine Projects' ongoing community outreach and provision of health screening and point-of-care services targeting vulnerable, medically under-resourced populations in collaboration with community and university partners. The Nurse Practitioner must be able to travel to community-based sites and encampments by operating the CDU Ford Cargo Vans and possess a valid California Class C Driver's License.Assumes ongoing responsibility for each patient from discovery of the disorder through recovery and referral to local primary healthcare facilities, including MLK Hospital Outpatient Clinic and Kedren Primary Healthcare Clinic.Performs complete physical examinations, obtains comprehensive medical histories, and orders, interprets, and evaluates diagnostic tests.Diagnoses and treats acute and chronic medical conditions within the Nurse Practitioner's scope of practice.Establishes, documents, and manages individualized treatment plans and maintains complete and accurate medical records.Utilizes the Electronic Medical Record (EMR) system to document all patient care and services.Initiates and manages drug therapy in accordance with California Nurse Practitioner regulations and collaborative practice requirements.Provides wound care and point-of-care testing, including diabetes screening, HbA1c, cholesterol, urinalysis, pregnancy testing, STI screening, and other appropriate preventive health services.Determines when patients require referral to primary care, specialty care, behavioral health, or emergency medical services.Participates in Quality Improvement and Quality Assurance activities of the Department of Family Medicine Patient-Centered Medical Home.Works collaboratively with physicians and other members of the CDU Street Medicine clinical team to coordinate, implement, and evaluate healthcare services provided at community-based outreach sites and encampments.Travels to designated community outreach locations and encampments by operating CDU Ford Cargo Vans as needed.Drafts monthly program reports.Qualifications/Requirements:Licenses:Current California Registered Nurse (RN) license.Current California Nurse Practitioner certificate issued by the California Board of Registered Nursing.Valid California Class C Driver's License with the ability to safely operate CDU Ford Cargo Vans for travel to community outreach sites and encampments.A minimum of two years experience in working with a mobile street medicine project serving under-served and medically under-resourced and uninsured populations of racial/ethnic minorities.Demonstrated ability to work independently in community-based and street medicine settings.Experience providing care to individuals experiencing homelessness and other vulnerable populations is preferred.Ability to work collaboratively with physicians, interdisciplinary healthcare teams, and community partners.Bilingual (English/Spanish) preferred.Complexity:The ability to perform highly detailed work with sustained attention and care while providing/obtaining information on numerous inquiries is paramount to success in this position.Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Work Environment:The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of a street medicine mobile outreach project.
