Home Care Shifts, Registered Nurse

Role Overview As a registered nurse, you will contribute professional nursing skills within an interdisciplinary team to deliver patient care through the nursing process.
Qualifications Licensed Registered Nurse (RN) with current license from the appropriate state licensing board
Basic Life Support (BLS) certification within 30 days of hire
Bachelor’s Degree in Nursing or Nursing Diploma
Key Responsibilities Assess patient status via interviews, observations, and physical assessments
Develop individualized care plans collaboratively with patients, families, and healthcare team
Implement interventions to meet patient outcomes and evaluate progress regularly
Document patient care accurately, including charts and reports
Operate and troubleshoot medical equipment, sharing knowledge with team members
Perform bedside lab testing adhering to policies and quality standards
Coordinate interdisciplinary care, utilizing resources effectively
Analyze clinical situations to resolve issues and meet patient needs
Prioritize tasks based on patient condition and assist team members as needed
Communicate effectively, including shift reports and advocating for patients
Educate patients and families using appropriate strategies and resources
Ensure safety and quality care aligned with standards, safety goals, and initiatives
Participate in quality improvement efforts and data analysis for process enhancements
Address patient and family requests promptly and serve on unit councils
