On-Call Hospice Registered Nurse (RN)The On-Call Hospice Registered Nurse (RN) is responsible for providing after-hours nursing support to hospice patients and families. This role focuses on responding to urgent patient needs, providing symptom management, and offering emotional support during evenings, nights, weekends, and holidays.The On-Call Hospice RN works closely with the hospice care team to ensure continuity of care and timely response to patient and family needs.QualificationsRequiredActive Registered Nurse (RN) license in the State of ConnecticutCurrent BLS certificationMinimum of 2 years hospice nursing experiencePreferredHome health experienceStrong clinical assessment and triage skillsWhat You’ll DoRespond to after-hours calls from hospice patients and familiesProvide clinical triage and nursing guidance over the phone when appropriateConduct in-person visits for urgent patient needs when requiredAssess patient symptoms and provide interventions to manage pain or discomfortProvide emotional support and guidance to patients and caregiversCommunicate patient updates to the hospice care team and document all interactionsCoordinate care with physicians and other members of the hospice teamEnsure continuity of care and timely follow-up for patient needsCompensationSalary: $500 per week + fee per visit
