Hospice Graduate Nurse

n Schedule: 8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation

• n As an RN Case Manager, you'll be the clinical anchor for your patients and their families — coordinating care, guiding the interdisciplinary team, and showing up when it matters most. Building and adjusting personalized care plans in collaboration with physicians and care teams \n

Administering medications and treatments per physician orders \n

Serving as the communication bridge between families, physicians, and community health partners \n

Participating in on-call rotation per team policy \n

Graduate of an accredited school of Nursing. \One (1) + year Hospice, home health or related organization experience preferred. \Compassion for patients and families navigating end-of-life care \n

Strong clinical judgment and the ability to work independently in the field \n
