We are a proud Joint Commission Accredited home health company.
We have an opening for a PER DIEM (field-based) California-Home Health Registered Nurse (RN) to provide skilled nursing visits to patients in Orange County and surrounding areas . This role requires strong clinical judgment, efficient documentation, and the ability to work independently in the field.
Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and routine skilled nursing visits.
Conduct comprehensive patient assessments and develop individualized care plans.
Perform medication reconciliation at each visit and report changes.
Provide patient and family education based on diagnosis and care needs.
Collaborates with Care Coordinators, and interdisciplinary team.
At least one year of nursing experience, including in a Home Health or similar environment is generally required (knowledge in OASIS documentation is a plus).
Being bilingual is a plus (Spanish, Vietnamese, Chinese).
Active California RN License .
Car Insurance.
Immunization Records ( Flu, Hep B, TDAP, MMR and Varicella ).
Flexible scheduling.
Supportive clinical and administrative team.
