Advanced Practice Provider - Primary Care (Part-Time)

Hourly Pay Range: $56.44 - $70.58. The hourly pay rate is determined by a candidate's expertise, years of experience, and other factors.

Advanced Practice Provider (APN/PA) - Primary Care, Family Medicine

Full Time / Part Time: Full Time, 40 hours per week

Hours: Monday - Friday, hours vary

Saturday rotation and call rotation required

Required Travel: No

Under the direction of collaborating physician(s), provides care to patients. Follows patients through their course of treatment in the clinic and hospital setting. Collaborates with physicians and the healthcare team to achieve optimal patient outcomes. Prescribes pharmacologic and non‑pharmacologic therapies as appropriate. Participates in the development, implementation and evaluation of standards of practice in the specialty. Employs strategies, interventions and teaching to promote, maintain and improve health. Provides comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease. Promotes wellness and prevention of illness and injury in various settings for individuals, families and communities. Positively contributes to safety outcomes and promoting high quality patient experience. Participates in the development, implementation and evaluation of service line goals. Adheres to all Endeavor Health policies and procedures.

Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required. Experience: Minimum 1 year APN/PA experience in Primary Care setting required. For APNs, Family Nurse Practitioners only.

Experience working in an outpatient family medicine setting.

Experience with both acute and chronic condition management and preventative care.
