Social Work Counselor
Hempstead, NY | Salary: $23.08/Hour | Schedule: Full-time, 35 hours per week
The Social Work Counselor will provide education and develop independent living skills in foster care children referred by the Department of Social Services.
Complete a Step Up Independent Living assessment and treatment plan in partnership with client, foster family and caseworker.
Complete and maintain client case records in compliance with agency and other regulatory standards.
Provide aftercare services to all PI program youth
Assume other tasks and responsibilities assigned to support the operations of the program
Bachelors degree in Human Service related field.
Proficient computer skills required.
