Night LVN for Home Health Care

Responsible for providing skilled nursing services to patients in their homes under the direction and supervision of a Registered Nurse. Must assess, plan, implement and evaluate patients needs and care. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards. Education: Graduate of an accredited practical nursing program; Consideration will be given for comparable education experience and successful licensure.

Current LPN license in the state of practice. Certification/Registration: Understands Hospice philosophy; Complies with Unity Health Hospice Health Status requirements. Is flexible and cooperative in fulfilling role obligation and has ability to work within an interdisciplinary team.

Employee is frequently to continuously alone with patient and must be able to handle emergencies without assistance.

Employee may be required to drive at night and/or in hazardous conditions; checking in with office daily to update schedule.

Employee has regular contact with agency staff members, physicians, hospital or medical center staff, general public.

Employee regularly handles confidential information.
