Registered Nurse / LVN Home Health/Hospice PRN

Pioneer Medical Center is a 25-bed critical access hospital, essentially the county hospital for Sweet Grass County, Montana: We serve our neighbors (ranchers, skilled trades people and active retirees) and the many tourists who come here on their way to Yellowstone and Glacier National Parks, or to fish the Yellowstone, hike, bike, hunt or just get away from overcrowded places. Our schools are highly rated, with high community support.
The PRN Hospice Nurse is responsible for the holistic nursing care of the patient. RN/LPN must provide hospice care within scope of practice and state regulations.
Supports and models behaviors consistent with Pioneer Medical Center’s mission, vision, values, code of business conduct and service expectations. Maintains competency in all organizational, departmental, and outside agency standards as it relates to the environment, employee, patient safety or job performance.
Provides skilled nursing care, patient visits, and assessments according to patient’s plan of care and needs.
Assists with picking up on-call shifts and providing patient visits if needed during scheduled call shift.
Assists with the admission process as directed by the Hospice Manager.
Implements nursing intervention based on nursing assessment of patient and assesses patient outcome.
Documents clearly and concisely in a timely manner, nursing care rendered, patient/family observations, teaching plans, consultations, etc.
Facilitates hospice transfer to inpatient setting, notifying hospital or skilled nursing facility admissions department/supervisor, and/or emergency room of hospice patient’s identity.
May accompany patient/family on transfer to inpatient setting, advocating patient’s desire for palliative care.
Answers patient/family questions and concerns relative to hospice care and services, condition changes, and plan of care.
Perform all necessary procedures and maintain skillful, compassionate care during the patients transition and for immediate post-mortem procedures. Notifies patient’s PCP/Medical Director of patient’s death.
Actively participates with the IDG in development of plan of care as appropriate.
Assists in educating, supervision, and counseling of patient/family regarding physical care, symptom management, and pain control.
Maintains competency in all organizational, departmental, and outside agency safety standards as it relates to the environment, employee, and patient/resident safety.

Regulatory standards (i.e., State Survey, OSHA)
Commitment to the hospice philosophy or care.
Effective verbal, nonverbal, and written communication skills, and leadership qualities.
Personal qualities enabling him/her to work effectively with patients, families, staff, and providers.
Utilizes proper body mechanics while performing all physical care. Ability to lift fifty (50) pounds and perform personal care on a patient who weighs up to two hundred fifty (250) pounds.
Physical and emotional health necessary for the accomplishment of the job.
A desire for continued personal and professional growth and development.
Incorporate population specific needs into all aspects of communication and patient care; Hospice nursing experience or an equivalent combination of education and experience relating to the above tasks, knowledge, skills, and abilities will be considered
Professional knowledge of nursing theory and practice acquired through an accredited school of nursing.
Current Montana state license to practice as a registered nurse or licensed practical nurse
Patient care visits in home, nursing home, and hospital settings.
