Haven Hospice is seeking a passionate, dedicated Hospice LPN to join our team in Lake City, FL. Our hospice LPNs provide expert, patient-centered care at a critical time in people's lives. Provide 1:1 care to make a lasting impact on patients and families

As a Hospice LPN You Will:

Give hands-on care to patients nearing the end of life, following state nursing rules.
Follow the care plan after assessing the patient's needs.
Keep clear and timely records of the care provided.
Teach and support patients and their families about the illness and care plan.
Help coordinate care by communicating with the hospice team (like aides, chaplains, social workers).
Take part in training and quality improvement activities to grow professionally and improve care.

*Graduate of an accredited practical nurse or vocational nursing program.
Three years nursing experience. Community health/hospice or medical/surgical experience is preferred.
Currently licensed as an LPN/LVN in the State.
