Responsible for demonstrating the Mission for Sunrise Senior Living, to champion quality of life for all seniors in accordance with federal, state and local standards and regulations and Sunrise Senior Living policies to promote the highest degree of quality care and services to our guests/residents./ph2Responsibilities Qualifications/h2h3Essential Duties/h3pAs a part of the Sunrise team, supporting our Mission, Principles of Service and Core Values is a fundamental part of this job. pulliDeliver care and services for designated group of guests/residents according to Individualized Service Plans (ISP)./liliReport all incidents immediately to appropriate supervisor / team member./liliReport suspected abuse immediately per regulations and Sunrise standards./liliParticipate in the care planning for their designated guests/residents/liliPerform individualized personal care and activities of daily living (ADL); documents ADL in flow sheets/liliPartner with licensed nurse to plan, develop, organize, provide and execute individualized restorative therapy programs, activities and standards for their designated guests/residents./liliObtain, document and report the guest/residents vital signs, urine output and meal intake per individualized care plan to licensed nurse as requested./liliObserve and report any physical and/or psychosocial changes of condition requiring a licensed nurse intervention and care services./liliAssist in maintaining assistive devices, adaptive equipment and other medical equipment to include cleaning equipment and making readily available./liliMaintain Restorative Records for each guests/residents./liliMaintains knowledge of federal, state/provincial and local resident care regulations/li/ulh3Quality Indicators/h3ulliStrive for excellent quality care and service delivery as measured in the Quality Assurance and Performance Improvement (QAPI) process./liliDemonstrate and knowledgeable of the following key quality improvement areas of the DNA/C.N.A. role:/liliADL, skin care, dining assistance, bowel/bladder elimination/liliFalls and fall prevention/liliSkilled nursing transformation and Resident Centered Care model/li/ulh3Guest/Resident, Family and Visitor Relations/h3ulliRespond to guest/resident, family, visitors inquiries, concerns in a timely and professional manner using the Problem Resolution process./liliInvite, Encourage and Assist (IEA) guests/residents to meals./liliAssist guests/residents when transferring or discharged by collecting belongings, escorting to transportation, other as assigned./liliGreet new guests/residents and escort to their room; orients guests/residents to room/call lights and community layout and provide activity calendar./liliInventory guest/residents belongings and labels at time of admission and ongoing there after./li/ulh3Risk Management and General Safety/h3ulliPartner with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements and promotion of Risk Management programs and policies;
