PART TIME DIETITIAN- M

We are partnering with a well-established healthcare organization focused on delivering high-quality, patient-centered care to older adults in the community. This part-time Dietitian role offers the opportunity to make a meaningful impact by supporting individuals in maintaining their independence, health, and overall quality of life. In this role, you will collaborate with a multidisciplinary team to assess nutritional needs, develop personalized care plans, and provide education to patients and their families. This position includes a mix of clinical and community-based work, including occasional home visits.

Develop and implement individualized nutrition care plans

Provide nutrition education and counseling to patients, families, and caregivers

Support disease-specific nutrition management (diabetes, heart health, renal care, etc.)

Inspect meals for compliance with prescribed diets and quality standards

Assist with menu planning, recipe development, and coordination with food vendors

Collaborate with physicians, nurses, social workers, and therapists to provide integrated care

Maintain accurate and timely documentation in the Electronic Health Record (EHR)

Participate in team meetings, caregiver groups, and community events as needed

Active Oregon Licensed Dietitian (LD) credential

Bachelor’s degree in Dietetics, Nutrition, Food Systems Management, or related field

Background in community-based care, home health, or long-term care settings

Familiarity with interdisciplinary team (IDT) models of care

Strong communication skills with patients, families, and care teams
