Home Health Caregiver Job Summary As a Home Health Caregiver with BrightCare HomeCare, you will play a vital role in providing compassionate and personalized care to individuals in their homes. Our team is dedicated to delivering high-quality services that cater to the unique needs and goals of each client. You will be responsible for providing direct patient care, maintaining a clean and safe environment, and collaborating with our multidisciplinary team to ensure the best possible outcomes.

Responsibilities Provide direct patient care, including assistance with activities of daily living (ADLs), such as bathing, dressing, grooming, and toileting

Maintain accurate and detailed records of patient care and progress

Implement and update patient care plans in collaboration with the healthcare team

Assist with medication administration, wound care, and other medical treatments as needed

Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination

Collaborate with our multidisciplinary team to develop and implement individualized care plans

Participate in continuous quality improvement initiatives to enhance patient care and outcomes

Requirements High school diploma or equivalent required

Certification in home health care or equivalent experience

Must be able to travel to and from client homes, as needed

Ability to work varied shifts, including days, evenings, and weekends
