Scheduling: Services shall be provided Monday through Friday between 0700 and 1530 for eighty (80) hours per week. Shifts greater than six-hour (6) hours include an uncompensated thirty-minute (30) meal break. Requirements for services may vary on holiday weeks. The government provided matrix shall reflect these changes. The clinic is closed on all Federal Holidays.
Medical Supplemental (Type I) Substitute Contract Worker: The place of performance will not require a substitute contract worker unless the permanent contract worker is absent for more than 3 calendar days.
Closures: During unplanned (unknown) closures due to no fault of the Contractor (e.g., natural disaster, military emergency, severe weather, pandemic, or Government decision to close the facility outside of a planned closure), the contract worker will be compensated for the unplanned closure (personal services only).
Telework Arrangements: Contract workers will not be capable of providing services in a telework environment.
Mission Essential: The following labor categories are designated as mission essential:
Licensed Practical Nurse.
Legal Authorization to Work in the United States: The contract worker performing under this contract must be a US Citizen.
Primary Responsibilities
General Duties:
Contractor personnel shall perform a full range of duties consistent with their labor category, education, training, experience, clinical privileges (as applicable) and assigned position. General duties that apply to all personnel or across certain labor categories are given below.
Maintain a level of productivity comparable with that of other individuals performing similar services.
Participate in peer review and performance improvement activities.
Practice aseptic techniques as necessary. Comply with infection control guidelines to include the proper handling, storage, and disposal of infectious wastes, and the use of universal precautions to prevent the spread of infection.
Function with an awareness and application of safety procedures.
Perform efficiently in emergency patient situations following established protocols, remaining calm, informing appropriate persons, and documenting events. Anticipate potential problems/emergencies and make appropriate interventions. Notify supervisor, director, or other designated person regarding problems that the HCW is unable to manage.
Apply an awareness of legal issues in all aspects of patient care and strive to manage situations in a reduced risk manner.
Participate in the implementation of the Family Advocacy Program as directed. Participation shall include, but not be limited to, appropriate medical examination, documentation, and reporting.
Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF.
Maintain an awareness of responsibility and accountability for own professional practice.
Participate in continuing education to meet own professional growth.
Attend and/or comply with all annual training classes required by the Command, to include but not limited to online annual training provided by the MTF: disaster training, infection control, Sexual Harassment, Bloodborne Pathogens, Fire and Safety, Chemical, Biological, Radiological, Nuclear and Explosives (CBRNE), and all other required training.
Actively participate in the command’s Performance Improvement Program. Participate in meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist.
Participate in the provision of in-service training to clinic staff members. Provide training and/or direction as applicable to supporting government employees (e.g., hospital corpsmen, students, etc.).
Attend and participate in various meetings as directed.
Perform timely, accurate, and concise documentation of patient care.
Operate and manipulate automated systems such as CHCS, AHLTA, Essentris, MHS Genesis and Clinical Information System (CIS), participate in clinical staff Performance Improvement (PI) and Risk Management (RM) functions, as prescribed by the Commander. Maintain DOD email account as directed. The HCW shall be responsive to all email and voicemail communications.
Exercise appropriate clinical delegation of tasks and duties in the direction and coordination of health care team members, patient care, and clinic activities.
Maintain documentation of all treatment provided in accordance with clinic directives, and prepare such records and reports as may be required. All records and reports must be legible.
Abbreviations must be only those listed in local instructions.
Core Duties:
The HCW shall provide services within the scope of their competencies, the applicable duties provided in the basic contract, and the following:
Ensure patient care is carried out in accordance with the Standards of Nursing Care and the policies and procedures of the clinic.
Check patients into the clinic and triage using clinic guidelines. Inform the nurse or physician of the patient’s condition and potential problems.
Obtain and document patient and pertinent family history.
Perform a full range of diagnostic support duties which include taking vital signs, collecting specimens, obtaining, recording and tracking results of diagnostic tools.
Assist the physician in a variety of diagnostic examinations such as lumbar punctures, colposcopies and sigmoidoscopies, by preparing, positioning and monitoring patients, and setting out and handling instruments and equipment.
Perform laboratory tests such as checking urine for sugar and stool for blood. Record and report results of test.
Perform a range of treatment procedures that include sterile dressing changes, applying compresses, monitoring IV fluids, inserting catheters and suctions, inserting nasogastric tubes, administering medications, giving enemas, setting up and giving treatment that requires auxiliary equipment such as oxygen and suction. Ambulate patients to other areas to include Radiology, various clinics, and physical therapy.
Prepare patients for tests, examinations, treatments, and/or surgery. Collect specimens such as urine, sputum and stool. Label specimens for laboratory examinations and follow up by using CHCS/AHLTA or calling the laboratory for reports. Alert provider to conditions which deviate from expected findings.
Prepare, start, monitor and discontinue intravenous fluids with accuracy and in accordance with established procedures.
Observe, report and document all observed symptoms, reactions, treatments and changes in the patient condition to the registered nurse or physician. Make careful observations to assess that nursing procedures and treatment do not cause additional distress.
Perform routine nursing care activities such as taking blood pressures, temperatures, baths and hygiene care, passing and removal of trays, changing of linen, and otherwise assist in the care of the physical needs of the patient.
Operate basic equipment required in delivery of patient care such as pumps, intravenous (IV) pumps, oxygen administration apparatus and incentive spirometers.
Execute physician’s orders within the guidelines of standard nursing practice. Ensure accurate medication is administered in correct form and dosage to the proper patient as directed by the physician.
Maintain records of nursing care, dose and time of medication administered, and indicate if the medication was not administered and the reason.
Administer immunizations in a safe and accurate manner with strict adherence to all applicable immunization policies.
Recognize emergency situations and assist with, or institute emergency measures for sudden adverse developments in patients such as cardiac arrests.
Perform preoperative procedures for minor surgery and fill out pre-op checklist.
Assist patients in admission, transfer, and perform discharge planning follow-up and documentation.
Support the patient and/or family members toward the achievement of treatment plan goals. Provide instructions to the patient on invasive procedures, surgical procedures and post-surgical conditions which were previously provided to the patient by a nurse or physician.
Instruct patients on how equipment is used such as oxygen, suction, cardiac monitor and pulse oximeter. Instruct patients and family on use of prescribed medications, contraindications of medications, and the necessity of proper follow-up care.
Provide emotional support to patients and families.
Ensure necessary supplies are available and equipment is in functioning order.
Provide an orderly, clean and safe environment for patients and staff.
Handle telephone information requests with courtesy, accuracy and respect for patient confidentiality. Receive information and distribute messages as necessary.
Report to supervisor any need for maintenance or re-supply of pharmaceuticals.
Ensure upkeep and perform checks of emergency equipment i.e., oxygen, emergency cart, suction apparatus, etc. and maintain appropriate logs. Inform the Charge Nurse of any discrepancy on daily checks.
Perform additional duties within the scope of LPN practice including but not limited to, update of Standards of Nursing Care, and other collateral duties required for efficient, quality patient care.
Transport patients to and from other areas of the facility to include hospital clinics and ancillary departments, as ordered by the physician.
Assist the professional nurse and/or physician with individual, group, and family health instruction in relation to the patient's condition and his/her ability for self-care. Assist in the dissemination of appropriate literature dealing with subjects related to the particular health problems involved. Assist in providing physical and psychological support to patients, assist in the explanation of procedures and treatment, and promote cooperation among staff, patients and families.
Initiate and maintain pertinent patient and family medical history on patients, recording observations and stated symptoms for use by the Medical Home Port (MHP) Team. Observe and record pertinent vital signs, determine and order laboratory procedures on patients that may be ordered by the provider to diagnose and treat the patient, per command and department policies. Orient new patients and their families to clinic policies as required.
Utilize automated programs and information technology tools to facilitate the patient experience and screen Medical Home Port patients, needing preventative services (mammograms, A1C, etc.). Ensure data entry for population health metrics and health promotion, while assisting in the maintenance and update of patient registries.
Utilize the patient’s preferred method of communication to provide integrative and comprehensive care and use clinic available tools for proactive patient management of their health care needs. (i.e., AHLTA, CHCS, Care Point, ICDB and Secure Messaging).
Track HEDIS measures, lab/rad results and notify patients per command/department policies. Check patient identification, physician's orders, and transfer information as applicable. Requisition and arrange for ordered diagnostic and therapeutic services as directed.
Ensure review of emergency treatment records (ETR) and follows up with patient, per command and department policies.
Administer immunizations, allergy treatments, oral, parenteral, IV medications, and maintains annual competencies as outlined in departmental policies.
Actively participate in MHP Team huddles. Proactively assists team’s identification of diagnostic procedures required during schedule scrub. Assist with identification of patient education needs and reviews plan with team members for coordination of educational needs.
Prepare equipment and assist physicians with treatment procedures.
Assist with clinic check-out and follow-up visits by reviewing post-visit needs with the patient.
Ensure scheduling of follow-up appointments or specialty care referrals. Assist patients by directing or escorting to other clinical services (laboratory, radiology, pharmacy, immunizations, etc.), as required.
Participate in the peer review and training of staff and maintenance of the Medical Home Port Team Accreditations (NCQA, JC, IG etc.).
Minimum Qualifications
Degree:
Education: Graduation from a State Board of Nursing approved LPN/LVN training program or completion of equivalent military training that permits sitting for the State licensure examination.
As an alternative to the education requirements in the basic contract, the HCW may be a graduate of a State Board of Nursing approved LPN/LVN training program OR be a graduate of an accredited LPN program by ACEN, OR have completed an equivalent military training program.
Experience: Possess a minimum of 1 year experience as an LPN working in an outpatient acute care (hospital based) or ambulatory care setting within the last two years.
Certifications (in addition to Basic Life Support) - None
Licensure: Possess a current unrestricted license to practice as an LPN/LVN in any of the fifty (50) States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.
