Job Title: RN Visit Nurse - Home Health | St. Paul Territory | $4,000 Sign-On Bonus

Location: St. Paul, MN and surrounding communities

Schedule: Full-Time with On-Call and Weekend Rotation

Compensation: $75,000 to $100,000 per year plus a $4,000 Sign-On Bonus

What You'll Be Doing Provide skilled RN home health visits throughout the St. Paul territory

Deliver wound care , medication management, injections, and disease-specific treatments

Educate patients and caregivers on medications, diagnoses, and recovery plans

Coordinate care with physicians, therapists, and interdisciplinary team members

Monitor patient progress and communicate changes in condition

Help patients remain safely at home while reducing avoidable hospitalizations

Who We're Looking For Active Minnesota Registered Nurse (RN) license

Experience in home health , hospital, rehabilitation, skilled nursing, or acute care

Strong clinical judgment and assessment skills

Excellent communication and patient education abilities

Self-motivated and comfortable working independently

Reliable transportation and willingness to travel throughout the St. Paul service area
