Job Title: Registered Nurse (RN) – Home Care Job Type: Full-Time Location: Are you a compassionate and skilled RN looking to make a real impact in your community? Join our dedicated team and deliver high-quality home care services that make a difference in patients' lives. We’re seeking a motivated Registered Nurse (RN) who thrives in a collaborative environment and values personalized care.
Conduct comprehensive initial assessments of patients and their families to determine care needs.
Develop and regularly update individualized care plans that reflect patients’ evolving conditions.
Implement preventive, therapeutic, and rehabilitative nursing procedures in the home setting.
Collaborate closely with physicians, caregivers, and other health professionals to coordinate care.
Supervise and evaluate care provided by home health aides every two (2) weeks.

- Active Registered Nurse (RN) license in Pennsylvania
- 1–2 years of acute care experience in a healthcare setting
