Fairview Health Services is looking for an RN at the University of Minnesota West Bank in Minneapolis, MN on our Transitional Care Unit! At M Health Fairview University of Minnesota Medical Center - West Bank Campus we are committed to providing exceptional healthcare in a setting where we specialize in innovation. Clinical trials advance our clinical services, bringing the very newest ideas in patient care.
Unit Details:
- Serves multiple diagnoses including neuro, ortho, oncology, amputee, spinal cord injury, transplant, etc.
- All adult patients (18+)
- Post-Acute - PT, OT, SLP.
- Approximately 50 therapy and tech staff at the 2 post-acute units
- Highly Collaborative between all teams along with MD/Physiatrist, rehab nursing, social work, dietary, etc. This position is working 64 hours in a 2-week pay period on the day/evening shift, 8 hour shifts and every other weekend. The position is eligible for Full benefits package, including rn student loan repayment.

Assesses patient needs and identifies expected nursing outcomes.
Involves the patient, family and other health team members in gathering data.
Uses an ongoing, systematic process to gather relevant information (biological, psychological, spiritual and social aspects) from patient and family.
Analyzes data and information to identify potential risks, prioritize needs and determine priority problems.
Validates these with patient, family and other health team members.
Documents data and identified outcomes.
Implements nursing interventions based on the plan of care.
Provides care according to the plan, and in accordance with all relevant policies, procedures and nursing practice guidelines.
Coordinates nursing care with the care of all other health team members.
Delegates direct and indirect patient care activities to appropriate personnel based on needs, condition, potential for risk/harm, complexity of the skill and predictability of the outcome.
Assures all actions are based on scientific, ethical or ecological principles; or, legal parameters.
Documents interventions according to practice guidelines.
Current Minnesota RN License
Bachelors Degree in Nursing or BAN
