JobSummary Employee is responsible for the care if the residents in adesignated area of the facility. Employee communicates with off going andarriving shifts of personnel to assure the continuum of quality of care. Makesrounds, evaluating the physical, emotional and social needs of the residentsmaking sure these are being met through the staff assigned to work with eachgroup off residents. Supervises the careprovided by the Nursing Assistants assigned to the unit, providing interventionand education as indicated. Consults with physicians, receives records and transcribes orders as required.Makes rounds with physicians when necessary. Reviews resident’s care plans andassures these plans are followed. Documents on records as per policy andprocedures of the facility. Participates in the evaluation of the employeesworking under his/her supervision. Education Employee must be a graduate of an accredited school ofnursing currently registered with the state agency for nursing licensure andhold a valid nursing license in the state he/she is employed.
Has the ability to organize, plan and supervise care and the Nursing Assistants assigned to work at a designated unit andshift.
Must be willing and capable of providingemergency assistance to residents as indicated.
Is courteous in working with residents, familiesand employees.
Maintains a net, well-groomed and professionalappearance at all times.
Must be willing to be in the facility ready tobegin assigned duties on time.
Mustbe willing to be in the facility during weekdays as well as weekend hours.
Mustbe willing to work preferred shifts as scheduled and assist on other shifts asneeded to assure adequate staffing to meet the needs of each resident.
Mustdemonstrate ability to evaluate the geriatric resident’s needs and provideintervention timely to assure each resident’s needs are met.
Mustdemonstrate ability to evaluate nursing staff needs and provide staff educationtimely to provide quality care.
Mustdemonstrate ability to function as a cooperative team member with each memberof the Nursing Department and other departments.
Mustdemonstrate the ability to follow and supervise assigned staff to assure thefacilities policies and procedures are followed and upheld.
Mustdemonstrate the ability to manage and supervise the assigned unit in aproductive, timely manner.
Receives report from the off going shift.
Reviews communication systems to beknowledgeable of any unusual occurrences during the previous shifts.
Assigns Nursing Assistants and other designatedpersonnel assuring each resident’s needs will be met during the assignedshifts.
Gives report to Nursing Assistants.
Makes rounds of the assigned unit frequentlyduring the shift providing intervention as needed. During the entire shift,will be attune to needs of unit while doing other nursing tasks.
Reorders medications as needed in preparation for future shifts. Assesses anddocuments medication issues as per policy.
Prepares a weekly skin report for the Director of Nursing. Supervises the meal service for each resident onthe unit. Works with the staff to sure each resident is provided adequate nutritionfor each meal.
Continuallyassesses and reassesses residents for risk of complications as a result ofbeing in the facility. (Continuallyobserves progress and quality of work of the Nursing Assistants to assure theresidents are turned, repositioned, toileted, walked, exercised, providedfoods, and fluids, well groomed, involved in their specific restorativeprogram, provides intervention as indicated, etc. Also observes the resident’senvironment to assure a clean and safe environment is provided for eachresident.
Documenton nurse’s notes as per policy. Reviewsinformation, reassess residents, interviews staff, reviews documentation ofeach shift and writes monthly nurse progress notes using the InterdisciplinaryCare Plan documenting progress or lack of progress toward the resident’s goals.Writes suggestions for changes to be made to the plan of care on the “WorkingCopy” of the Interdisciplinary Care Plan
Notifies the physician of results.
Notifiesthe physician of unusual findings as indicated.
Reviews,signs and provides intervention as indicated for Nursing Assistantsdocumentation.
Countsrequired medications with oncoming and off going shifts.
Makesappropriate notes in Communication Books to assure adequate communication ismaintained for nursing.
Assistsin keeping other departments informed of resident’s change of condition asindicated.
Assigns Nursing Assistants to keep eachancillary room clean and orderly. Assigns staff to keep equipment clean andproper condition for use.
Reviewsand updates Medication Administration Record, Treatment Administration Record,and Nursing Care Flow Records for the upcoming month.
Makesrounds toward end of shift to assure area is in order for on coming shift.
Givesreport to oncoming shift.
Reviewand assures there is adequate staffing for the oncoming shift. Performsany other tasks as assigned by the Director of Nursing or Assistant Director ofNursing.
Worker will be bending and stooping toassist residents, co-workers, etc. Worker will be assisting or totally liftingresidents as each resident needs, some residents needing to be totally liftedor limbs may be lifted. Worker will be lifting various equipment for resident’sneeds, etc. Worker would be expected to lift, with assistance, up to 300 poundsusing equipment or other personnel to assist in the lifting.
Worker will be carrying equipment, etc. Worker will be pushing or pulling medicationcarts, equipment, etc. to provide care. Worker will be balancing when assistingresidents to transfer or transport, giving medication, doing treatment,providing care, etc.
Worker will be twisting or turning whenproviding medication and treatment systems, when assisting residents totransfer or transport, assisting staff to provide for Activities or DailyLiving.
L.s, gather equipment, provide care forthe residents, etc.
Reaching: Worker will be reaching when providingmedication and treatment systems, when assisting residents to do A.D.L.s,gathering equipment, providing care for the residents, etc. Must communicate with residents, co-workers,families, visitors and physicians continuously throughout tour of duty.
JOB LOCATION Must work throughout the nursing areas of the facility, Must work throughout the nursing areas of the facility, May requireworking irregular hours and may be exposed to communicable diseases and/or bodyfluids, medical preparations, toxic substances and occasionally ionizingradiation.
