Active LPN or RN License issued by NYS

Minimum of 6 months experience in a home care setting.

Empathy, Reliability , and a commitment to high-quality care

What you'll do : Perform 1:1 patient care duties and treatments as per care plan.

Provide health promotion, counseling, and education.

Coordinate care, in collaboration with a wide array of health care professionals.

Document nursing notes regarding patient care daily.
