What You'll Do

Evaluate residents' functional abilities and therapy needs
Create and implement individualized treatment plans
Deliver skilled physical therapy services per physician orders
Document treatments and resident progress accurately and timely
Collaborate with our interdisciplinary care team
Educate residents, families, and caregivers
Ensure compliance with all regulatory and facility standards
What We're Looking For

Active Kansas Physical Therapist license in good standing
SNF or long-term care experience preferred
Strong communication and interpersonal skills
Familiarity with documentation systems a plus
Commitment to compassionate, high-quality care
