Monitor patient conditions and communicate changes to the care team
Educate patients and caregivers on care plans and treatments
Collaborate with physicians, case managers, and interdisciplinary healthcare professionals to ensure quality patient care
Active Arizona LPN License (or Compact License if applicable)
Valid Driver's License with reliable transportation and current auto insurance
Arizona Fingerprint Clearance Card
Article 9 Training (provided during onboarding)
