TravelNurseSource is working with Conflux Systems Inc to find a qualified Home Health RN in Rutland, Vermont, 05736!
We are seeking an experienced Home Health Registered Nurse (RN) with strong OASIS documentation experience to provide skilled nursing care, case management, and coordination of services for patients in the home setting. This role is responsible for comprehensive patient assessments, development and implementation of individualized care plans, patient/caregiver education, interdisciplinary collaboration, and timely regulatory documentation. Utilize the nursing process to assess patient and caregiver needs and provide quality skilled nursing care in the home setting.
Develop, implement, and communicate individualized plans of care in collaboration with patients, caregivers, and providers.
Perform skilled nursing interventions including medication management, wound care, IV therapy, catheter care, injections, and disease management.
Maintain competency with high-tech nursing skills such as IV use/maintenance, wound VAC, and complex wound care.
Monitor patient response to treatment and revise care plans as needed.
Collaborate with LPNs, Home Care Aides, therapists, social workers, and other team members to ensure continuity of care.
Delegate tasks appropriately within scope of practice and provide ongoing supervision as needed.
Educate patients and caregivers on medications, side effects, treatment options, reportable symptoms, discharge planning, and available services.
Coordinate care with physicians, providers, schedulers, vendors, and interdisciplinary team members.
Participate in case conferences, IDT meetings, peer review audits, and quality improvement activities.
Report incidents, infections, injuries, and safety concerns per policy.
Coordinate safe patient discharges, transfers, and transitions of care.
Advocate proactively for patient and family needs to ensure quality, individualized care.
Maintain infection prevention and safety standards throughout the plan of care.

Current Registered Nurse (RN) license in applicable state.
Graduate of an accredited School of Nursing.
Previous Home Health nursing experience required/preferred.
Valid driver’s license and reliable transportation required (if field-based role).
Experience with McKesson or similar home health documentation systems.
Experience with hospice or interdisciplinary team-based community care preferred.
Strong wound care, IV therapy, and chronic disease management experience preferred.
Strong assessment and case management skills.
Excellent knowledge of OASIS and regulatory documentation.
Compassionate, detail-oriented, and patient-centered approach to care.
Patient homes and community-based settings.
Independent field work with frequent travel between visits.
Direct patient care involving standing, walking, lifting, and clinical procedures.
