Occupational Therapist
Plans, implements, and evaluates client care plans to restore or maintain patient well-being. Evaluates occupational roles and performance of clients with physical, psycho-social and/or cognitive impairments. Document, prepare/revises and completes a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation on an ongoing basis.

• Apply concepts of infection control and universal precaution in coordination/performing client cares activities to protect both client and staff (OSHA).
• Assess and facilitate through direct therapeutic intervention client’s ROM, strength, coordination needs for and use of adaptive equipment and environmental modifications, functional mobility and safety issues as related to client’s ADL performance needs.
• Provides effective communication to clients, their families, team members, and other health care professional, and regular participation in Case Conference/Team Meetings. Evaluate client occupational performance in the areas of self-care, work, leisure, and rest. Completes appropriate documentation in a timely manner to assure compliance with company policy.
• Preferred, home health experience.

Therapy skills as defined as generally accepted standards of practice.
Must read, write and comprehend English.
Bachelor of Science in Occupational Therapy.

Reliable transportation and auto liability insurance.
On-going employment as an OT always requires maintenance of a valid OT license in the state which the therapist practices in and a valid CPR certificate. Environmental/Working Conditions:
Possible exposure to blood borne pathogens, bodily fluids and infectious diseases.
PRN emergency call.
Ability to travel locally and work a flexible schedule.
