We provide home health, hospice and community-based services that help people receive quality care where they feel most comfortable: at home. Rooted in personalized care, we support patients and provide comfort through each stage of their health journey, while building trusted relationships along the way. The Physical Therapist - HAC is responsible for helping patients restore function, improve mobility, and enhance overall quality of life. This role provides skilled physical therapy services under a physician-directed plan of care, including assessment, treatment planning, intervention, and care coordination within an interdisciplinary care model.

Conduct comprehensive patient assessments and develop individualized physical therapy plans of care in collaboration with physicians

Provide skilled physical therapy interventions and modify treatment plans based on patient progress and clinical needs

Perform ongoing reassessments, including weekly evaluations, to support care planning and physician coordination

Maintain accurate, timely documentation in compliance with clinical, regulatory, and documentation standards

Current, unencumbered license to practice physical therapy in the assigned state

• Graduate of an accredited physical therapy program (CAPTE accredited)
• 1+ year of experience in physical therapy; 1+ year of experience in home health, hospital-at-home, or community-based therapy settings

1+ year of experience working within interdisciplinary healthcare teams

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $106,400 to $159,600 annually based on full-time employment. We comply with all minimum wage laws as applicable.
