Full Time Physiatrist Needed

Physical Therapist (PT) – Full-Time | Home Health – Jackson, TN

Step beyond the traditional clinical setting and provide one-on-one care—helping patients recover, manage chronic conditions, and maintain independence in the comfort of their own homes.

• Provide physical therapy services according to a physician’s plan of care and state practice guidelines

Evaluate patient function and assist in developing individualized care plans

Develop recommendations for modifying equipment and home environments to support patient safety and independence

Collaborate with physicians and interdisciplinary team members to coordinate care and monitor patient progress

Document clinical notes, progress reports, and summaries in a timely and accurate manner

Support and motivate patients and families throughout the rehabilitation process

Supervise Physical Therapist Assistants in accordance with company policy and state regulations

Maintain accurate clinical records and track patient goals and outcomes

Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA)

Current Physical Therapist (PT) license in the state of practice

At least two (2) years of experience in an acute care or similar setting (preferred)

Current CPR and First Aid certification
