Provide personal care and assistance with daily living activities.
Administer medications and monitor client health as directed.
Assist with mobility and physical exercises to enhance client well-being.
Document and report changes in client condition to the healthcare team.
Support clients with meal preparation and nutrition guidance.
Engage clients in social and recreational activities to promote mental health.
Communicate effectively with clients, families, and healthcare professionals.
Current Home Health Aide certification in New York State.
Previous experience in home health care or a related field.
Ability to work flexible hours, including evenings and weekends.
Compassionate demeanor with a commitment to client care.
Reliable transportation to travel within Nassau County.
Basic understanding of medical terminology and procedures.
Tradition Home Health Care has been serving the Brooklyn community for over a decade, providing exceptional care and support to families in need. Compensation: $19.65 - $22.00 per hour
