Home Health Registered NurseCelebrity Home Health, Inc. is seeking a skilled and compassionate Registered Nurse (RN) to provide high-quality, patient-centered care in the home setting. The RN is responsible for comprehensive patient assessments, development and oversight of care plans, and coordination of services to ensure optimal patient outcomes.This role plays a critical part in delivering safe, effective, and compliant home health services in accordance with physician orders and regulatory standards.Key ResponsibilitiesClinical Assessment & Care PlanningPerform initial Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessmentsDevelop, implement, and update individualized Plans of Care (POC) in collaboration with physiciansConduct comprehensive patient assessments, including physical, psychosocial, and environmental evaluationsPatient Care & Case ManagementProvide skilled nursing care, including wound care, medication management, and disease educationServe as the primary case manager for assigned patientsMonitor patient progress and adjust care plans as neededEnsure coordination of care with therapists, LVNs, aides, and other disciplinesDocumentation & ComplianceComplete accurate and timely OASIS documentationMaintain compliance with Medicare Conditions of Participation (CoPs) and state regulationsEnsure all documentation meets agency, regulatory, and reimbursement requirementsUtilize the agency's EHR system effectivelyCare CoordinationCommunicate regularly with physicians regarding patient status and ordersParticipate in interdisciplinary team meetings and case conferencesEnsure continuity of care across all servicesPatient & Family EducationEducate patients and caregivers on diagnoses, medications, treatments, and disease managementPromote safety, fall prevention, and independence in the home environmentProvide clear instructions for ongoing care and follow-upQualificationsActive Registered Nurse (RN) license in the State of NevadaGraduate of an accredited nursing programMinimum 1–2 years of nursing experience (home health preferred)Strong clinical assessment and critical thinking skillsExcellent communication and organizational abilitiesAbility to work independently and manage a caseloadPreferred QualificationsHome health experience strongly preferredExperience with OASIS documentation and Medicare billing requirementsFamiliarity with home health EMR systems (e.g., Axxess, WellSky)Bilingual (English/Spanish) is a plusPhysical RequirementsAbility to travel to patient homes within assigned territoryAbility to lift, assist, and reposition patients as neededAbility to perform physical tasks such as standing, walking, bending, and reachingWork EnvironmentField-based position providing care in patients' homesFlexible scheduling based on patient needs and caseloadIndependent work with strong clinical support from leadership
