Full Time Physiatrist Needed

BoldAge PACE is seeking a dedicated Physical Therapist to join our Healthcare & Medical Services team, providing comprehensive rehabilitation services to older adults enrolled in our Program of All-Inclusive Care for the Elderly. In this role, you will focus on helping seniors maximize their independence, safety, and quality of life through evidence-based physical therapy delivered across a variety of settings, including our center, participant homes, and the community.

As a Physical Therapist with BoldAge PACE, you will perform thorough evaluations of each participant’s functional mobility, strength, balance, endurance, and pain levels, and translate your findings into individualized, goal-oriented treatment plans. You will design and implement therapeutic interventions that may include therapeutic exercise, balance and gait training, transfer training, fall prevention strategies, pain management approaches, and assistive device training. You will collaborate closely with an interdisciplinary care team that includes physicians, nurses, social workers, occupational therapists, and other healthcare professionals to coordinate and integrate therapy services into holistic care plans. Through regular communication and participation in team meetings and care conferences, you will advocate for functional goals, share clinical insights, and help shape comprehensive care strategies tailored to each participant’s needs.

You will conduct home and environmental assessments as needed to identify safety risks and recommend modifications that improve functional independence.

Accurate, timely documentation in the electronic health record is an essential part of your responsibilities. You will record evaluations, daily treatment notes, progress updates, and discharge summaries in accordance with professional standards and regulatory requirements.
