Job Opportunity
Position Overview
Seeking a dedicated Home Health Therapy position that includes home visits and periodic office meetings. Candidates must have reliable transportation, a current driver's license, and agency-required liability insurance.
Schedule
Shift: Day, 5x8-Hour (08:00 AM - 05:00 PM)
Contract Length: 13 weeks
Average Number of Patients per Day: 5-6
Weekly Productivity Expectations: 30 points (equates to approximately 40 hours of work)
Job Responsibilities
Conduct home health visits to patients in their homes and care facilities.
Complete all documentation within 24 hours of the appointment.
Accept all patients on the assigned schedule, regardless of distance from previous appointments.
Travel up to 75 miles from the assigned care center, with mileage reimbursed at the IRS standard rate.
Qualifications
Minimum of 1 year of adult home health experience required.
Must hold the necessary certifications: CPR and BLS.
Proficiency in OASIS and HomeCare HomeBase EMR systems required.
New graduates are not considered; recent home health experience is mandatory.
Miscellaneous
Must be comfortable with the possibility of long drives between patients.
Local candidates are welcome to apply.
No guaranteed hours; the contractor will be expected to engage with available patients appropriately to meet weekly productivity standards.
