$We are seeking a compassionate and clinically strong Consultant Pharmacist to join our team in supporting a PACE (Program of All-Inclusive Care for the Elderly) program in Mt. As a key member of the care team, you will help optimize medication therapy and improve health outcomes for older adults with complex medical needs.
Participate in interdisciplinary team (IDT) meetings and care planning
Conduct medication reconciliations and chart reviews
Ensure compliance with CMS, state pharmacy laws, and PACE guidelines
Willingness to travel
