Homecare Registered Nurse (RN) / Travel Nurse (Home Health)
Weekly Estimated Pay: $2,120 - $2,230
Location: Lake Charles, LA (serving a 50-mile radius) — Lake Charles healthcare jobs, Louisiana home health jobs
Shift Schedule: Monday–Friday, 8:00 AM – 4:30 PM
On Call/Weekend: 1 weekend per month and 1 day per week (on-call rotation)
Travel: Mileage reimbursed between patient visits (travel nurse/home health travel)
Active Registered Nurse (RN) license in Louisiana
Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
Minimum of 1 year of nursing experience (home health experience preferred)
Experience with HCHB (Homecare Homebase) and familiarity with LHC Group systems is a plus
Strong clinical assessment, documentation, and care coordination skills

Provide skilled nursing and clinical care for adult patients (18+) in a home health setting
Perform comprehensive patient assessments, develop individualized care plans, and implement skilled nursing interventions
Administer medications, wound care, disease management, and patient/family education as appropriate
Coordinate care with interdisciplinary teams, home health aides, physicians, and families to ensure continuity of care
Document patient care accurately and timely using electronic health record systems (EHR/HCHB)
Participate in on-call rotation and scheduled weekend coverage
