Physical Therapist (PT) - Health Coverage

Interim HealthCare is seeking a dedicated Home Health Physical Therapist for part‑time or full‑time opportunities serving adult and geriatric patients in and around Columbus, IN. As a key member of our Healthcare & Medical Services team, you will provide one‑on‑one, patient‑centered physical therapy in the comfort of each patient’s home, helping them regain function, improve mobility, and maintain independence.

Key Responsibilities

Conduct comprehensive physical therapy evaluations.

Develop individualized plans of care.

Deliver evidence‑based therapeutic interventions tailored to each patient’s goals and home environment.

Educate patients and family caregivers on exercise programs, safe mobility techniques, use of assistive devices, and fall‑prevention strategies.

Reassess progress regularly.

Update care plans.

Document all care in a timely and accurate manner.

Communicate effectively with the broader interdisciplinary team, including nurses, physicians, and other therapists.

You will manage a home‑health caseload, plan and prioritize daily travel to patient homes in the Columbus, IN area, and maintain clear, professional communication with patients, families, and colleagues. You will practice in a culture that values compassion, respect, and clinical excellence, with strong support from leadership, office staff, and fellow clinicians.
