Liaison RN - Hospice - Care T

SouthernCare New Beacon Hospice is seeking an RN Clinical Liaison to connect patients, families, and referral sources with compassionate hospice care. In this field-based role, you will assess clinical appropriateness for hospice, educate patients and families on services, and coordinate smooth transitions of care. Partner closely with physicians, hospitals, and facility staff to identify hospice-eligible patients and facilitate timely admissions. The ideal candidate is an experienced RN with strong assessment, communication, and relationship-building skills, committed to dignity, comfort, and support at the end of life.
