Commitment Advocacy Respect ExcellencePosition & Responsibilities:Under the supervision of the Physical Therapist, assists in evaluation and development of the rehabilitative plan of care.Practices and maintain patient safety and dignity by abiding by Federal, State, Local and Optimal policy & procedures as pertains to patient protected health information and HIPAA (Health Information Portability and Accountability Act).Assist patients in use of self-help devices.Participates in therapy and other in-service programs.Attends rehabilitation department staff meetings.Complies with agency Policies and Procedures as well as Employee Handbook detailing Human Resources policies.Completes emergency preparedness training as identified as appropriate for this employee level.Participates in exercises and drills for emergency preparedness, as required.Other duties and tasks as assigned by the Optimal Home Care Emergency Command Center communication tree in the event of a local, regional or national emergency and/or disaster.Why Optimal Home Care:Family Oriented CompanyDedicated intake and scheduling team that support you and your patientsSupportive work environment with opportunity to learn and developMileage reimbursementCompetitive benefits package401K plan + 15% matchingCompany phone providedPTOOpportunity to provide individualized care to patientsContinuing education opportunitiesiPad Purchase ProgramRequirements:Preferably One (1) year of experience.Graduated from a physical therapist assistant curriculum approved by the Commission on Accreditation in Physical Therapy Education of the American Physical Therapy Association or has passed a national examination for physical therapist assistants.Must be able to communicate in the language of the client and document client related activity as appropriate.
