Overview Office Location: Palatka, FL Coverage area: Schedule: 9-6 or 10-7 with alternating weekends

Are you a Registered Nurse looking for a new opportunity? Havenis seeking a passionate, dedicated Hospice Admissions RN to join our team in Jacksonville, FL. Our hospice RNs provide expert, patient-centered care at a critical time in people’s lives. Provide 1:1 care to make a lasting impact on patients and families

As a Hospice Admissions RN You will:

Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care

Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents

Obtain medical history, current medications, and symptoms

Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines

Coordinate the start of services (nursing, aides, equipment, medications.)

*Qualifications Graduate of an accredited nursing school with current licensure to practice in state of operation

One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
