Provide assistance with activities of daily living (i.e., personal hygiene) as part of the agreed-upon plan of care

Understand and apply basic infection control principles and procedures

Keep a record of HHA Plan of Care for each patient assigned and provide updates to assigned case manager and patient’s family

Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care

A Home Care Aide certificate

A minimum of one-year full-time experience in direct client care in an institutional setting (hospital or nursing facility)

One-year full-time experience within the last five years in direct client care in an agency setting
