We are looking for an experienced RN Navigator to join our team!
Hours: Monday - Friday - 7:30am - 4:00pm
Location - Homebase Obici Hospital - ability to work remotely - Supports all of Hampton Roads, Northern Virginia, and Northeast North Carolina.
They look at diagnostic codes and determine if they meet criteria for the screening, help get them scheduled, and support navigation during follow up care. We are looking for an RN who is detail oriented, a self-started with good follow through.
Responsible and accountable for the provision and facilitation of comprehensive care coordination services and quality outcomes for patients across the continuum. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation of patient/caregiver needs. Acute Care, Service Lines, Ambulatory/Community-based, Home Health, and Long Term Care.
Nursing Degree Required - BSN, ADN, or Diploma

• RN License is required in Virginia Multi-State.
• 3 years of RN experience required . Preference is experience in oncology, pulmonary, or thoracics.
• Specialty certification in case management or related area highly preferred. (3 years Case Management experience preferred.

The base pay range for full-time employment is $89,544.00 - $136,468.80 annually.
Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
