You will interact closely with patients and their families, guiding them through every step of their recovery journey and helping them restore their health within a real-world home context.What We OfferWe make it easy to do your job while offering competitive financial incentives and support:Compensation: Guaranteed base salary + generous, uncapped bonus structure.Travel & Bonuses: Mileage reimbursement plus additional bonus opportunities.Flexible scheduling with team support from scheduling to patient admissions.Comprehensive Benefits: Medical, dental, and vision insurance options tailored to your needs.Additional Perks: Paid time off (PTO), 401(k) eligibility, company-paid life insurance, and access to a robust Employee Assistance Program (EAP).Key ResponsibilitiesDirect Patient Care & Care PlanningProvide skilled physical therapy services in the patient's home as ordered by the attending physician.Develop, evaluate, and adjust individualized Plans of Care (POC) based on patient condition and progress.Treat patients to relieve pain, restore function, and maintain maximum performance using physical modalities (exercise, massage, heat, water, etc.).Front-load visits for high-risk patients to help reduce re-hospitalizations.Arrange outpatient therapy services when treatment cannot be delivered in the home.Instruct patients and families on proper equipment use (wheelchairs, braces, walkers, crutches, canes, etc.).Collaborate with the DME department and/or MSW to ensure families understand financial responsibilities for equipment.Supervision, Documentation & AdministrationOversee and supervise therapy services provided by PTAs and Nurse Assistants; Start of Care visits: Documented within 48 hours.Documented within 24 hours.Perform chart audits at agency sites as requested.Participate in case conferences, orientation/training of new employees, and required in-service meetings.Qualifications & AttributesLicensure: Active Physical Therapist (PT) license in the state of employment.Valid driver’s license and active auto insurance (with your name listed as a driver).Experience:Required: Minimum of 1 year of PT experience in an acute setting.Preferred: Prior home care experience.Knowledgeable in therapy best practices and compliant with all legal rules/regulations.Physical & Health RequirementsHealth & SafetyMust pass physical position requirements and be free of conditions that restrict job performance.TB Testing: Evidence of a current negative tuberculin skin test/x-ray required at orientation, followed by annual testing (unless prior documentation is provided).Physical DemandsConstant (66%-100%): Driving, computer/PDA usage, reading, writing, sitting, standing, walking, bending, reaching/stretching, speaking, seeing, and hearing (in-person and via phone).with or without assistance), climbing, stooping (bending at waist), and hand/finger dexterity.INDKHCWe’ll help you put your passion for patient care to work.
